We have on
this date entered into an agreement with Lines of Communication
to act as our communications representative. Under the terms
of this agreement and by this letter, we do hereby authorize Lines
of Communication to inquire and negotiate any billing matters,
traffic studies, handle all negotiations for service and/or equipment,
and to issue orders or execute contracts on our behalf and that
we are solely responsible for payment on said contracts and accounts
per the provider's specifications. We hereby authorize the chosen
carrier to perform any credit investigation required in conjunction
with providing the service.
By this letter, we affirm that we agree to comply with all terms
and conditions of said contracts, including but not limited to
Service Level Agreements, rates, fees, and provider terms and conditions
of service.
This authorization does not prevent our company from acting on
its own behalf when it is necessary.
